Fani na całym świecie śledzili losy pracowników i pacjentów chicagowskiego szpitala, by wspólnie przeżywać ich sukcesy i porażki oraz poznawać nowych członków personelu.
Pomysł realizacji serialu E. R. Ostry dyżur narodził się na początku lat 90 ubiegłego wieku. Początkowo Michael Crichton planował realizację filmu kinowego, którego reżyserią poważnie zainteresowany był Steven Spielberg. Niestety, z różnych powodów obraz nigdy nie powstał. Spielberg we współpracy z Crichtonem zrealizował Park jurajski, a E. R. Ostry dyżur ostatecznie zaistniał jako serial telewizyjny.
